Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated on January 18, 2023 and concluded on January 18, 2023. There were 44 children present with 9 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the vendor did not ensure that each staff complete annually the Virginia Annual Health and Safety Update course sponsored by the department as required.

Evidence:
1. The record of staff #1 (employed: 08/01/18) had documentation of the training from 09/16/22.
2. The record for staff #5 (employed: 05/27/20), staff #7 (employed: 09/05/18), and staff #8 (employed: 12/09/19) did not contain documentation of the Virginia Annual Health and Safety update.
3. The vendor acknowledged that the training update had not been completed annually as required.
