Inspector Notes
An unannounced monitoring/renewal inspection was conducted July 25, 2023 approximately 11:41 am to 1:00 pm. There were 7 children for a total of 21 points directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, observation of handwashing as transition to lunch. There was an assortment of age appropriate materials for children. There were ample learning material and toys for the children's learning development.

4Evidence:
1. Provider had Staff #2 as a substitute provider during the inspection today. The documentation forms in staff's record had another facility name listed. The provider did not have documentation forms for the name of this family day home in the staff's record.
Evidence:
1. Household member #1 did not have documentation of an initial TB screening.
The household member do not come inside daycare usually. She help me to carry the red box.
Evidence:
1. Household member #2 TB screening expired on 11/15/2022.
2. Household member #3 TB screening expired on 10/21/2022.
3. Household member #4 TB screening expired on 5/24/2020.
4. Household member #5 TB screening expired on 4/17/2022.
Evidence:
1. A blue mat used for indoor play had tears and rips along the sides of the material.
