During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 16, 2025 and completed on April 22, 2025. The on-site inspection began at 11:25 am and ended at 1:00 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 17. The inspector reviewed 2 children?s records and 2 caregiver/household member(s) records on-site on April 16, 2025.

This inspection included document review, tour of the facility, and observations.Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 29, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-520-A
Cribs shall be provided for children from birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot, rest mat, or bed. Child A was sleeping in a bouncer.
Plan of Correction: I will use crib for 12 months old and the children not developmentally ready to sleep on cot.

When food is brought from home, the food container shall be clearly labeled in a way that identifies the owner. Three children food mats were not labeled to identify the owner.
Plan of Correction: The food will labeld with child's name.
