Inspector Notes

An annual subsidy health and safety inspection was conducted on 3/14/2024 between 10:35 and 11:10am. There were three children receiving care by the vendor. The home, polices and procedures, emergency supplies and procedures, emergency drills, 3 children's records, the vendor/caregiver's record, and 2 household member's records were reviewed. An exit interview was conducted with the vendor. A violations was found on subsidy health and safety requirements and is identified on the violation notice.

Based on record review and interview, the caregiver did not have up to date tuberculosis screening results.
Evidence: Most recent TB screening dated 7/20/2021.
Plan of Correction: Vendor stated that the TB screening form is at her doctor's office. Will forward to Licensing, once completed.
