Inspector Notes

A monitoring inspection was conducted on July 19, 2022 and concluded on July 19, 2022. On July 19, 2022, the licensing inspector was onsite from approximately 10:15 am until 12:44 pm. The director assisted with the inspection. There were 26 children present, ranging in ages from 22 months to ten years, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and ten staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of ten staff records and interviews, the center did not ensure that one staff member had a central registry finding within 30 days of employment.

Evidence:

The record for Staff #3, hired on 05/09/2022, did not contain the results of a central registry finding. The record did not contain documentation of any further contact and the staff has been continuously employed. The director stated a follow up to the status of the request had not been conducted.
Plan of Correction: Per the Director: Going forward, I will call and verify the central registry request if results are not back within 30 days of hire. I will document the follow up for the record. The request was returned to us on June 27, 2022 for being incomplete and we mailed it back today.
Standard 8VAC20-780-160-A
Based on review of ten staff records, the facility did not obtain from one staff member documentation of a negative tuberculosis screening at the time of employment and prior to coming in contact that had been completed within the last 30 calendar days of the date of employment.

Evidence:

The record of staff #3, hired 05/09/2022, had documentation of a tuberculosis screening dated 03/10/2022, exceeding 30 calendar days prior to hire.
Plan of Correction: Per the Director: Going forward, we will have staff obtain a tuberculosis screening at the time of hire.

Based on observation and interview, the facility did not maintain the staff-to-child ratio of one staff for every ten three year olds up to school age.

Evidence:

The three year old classroom had 11 children in attendance with one staff exceeding the requirement. The director stated additional staff for the classroom had called out today.
Plan of Correction: Per the Director: We had staff call out sick today. I have corrected it by having another staff assist in the classroom.
