Inspector Notes
A monitoring inspection was conducted on July 19, 2022 and concluded on July 19, 2022. On July 19, 2022, the licensing inspector was onsite from approximately 10:15 am until 12:44 pm. The director assisted with the inspection. There were 26 children present, ranging in ages from 22 months to ten years, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and ten staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
The record for Staff #3, hired on 05/09/2022, did not contain the results of a central registry finding. The record did not contain documentation of any further contact and the staff has been continuously employed. The director stated a follow up to the status of the request had not been conducted.
Evidence:
The record of staff #3, hired 05/09/2022, had documentation of a tuberculosis screening dated 03/10/2022, exceeding 30 calendar days prior to hire.
Evidence:
The three year old classroom had 11 children in attendance with one staff exceeding the requirement. The director stated additional staff for the classroom had called out today.
