Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 1/20/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 17 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview, the child day program was not equipped with at least one carbon monoxide detector.
Evidence: The program owner stated that they have not yet installed a carbon monoxide detector.
Plan of Correction: Monitors have been plugged in as of inspection.
Standard 8VAC20-780-160-A-1
Based on record review, each staff member did not submit documentation of a negative tuberculosis (TB) screening prior to coming into contact with children.
Evidence:
1. Staff #1 (date of hire: 12/9/2020) TB screening is dated 1/6/2021.
2. Staff #2 (date of hire: 10/21/2021) TB screening is dated 11/12/2021
Plan of Correction: Will assure an updated TB test is in file before they come in contact with children.
Standard 8VAC20-780-240-A
Based on record review and interview, not all staff have completed the Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
Evidence: Staff #3 (date of hire: 6/3/2019), Staff #4 (date of hire: 9/7/2020) and Staff #5 (date of hire: 9/7/2021) records do not contain documentation of the required Virginia Preservice orientation training. The program owner stated that not all staff have completed the required training within 90 days of employment.
Plan of Correction: Providers will take the training by 2/28/2022.
Standard 8VAC20-780-240-C
Based on interview, staff have not completed all required training.
Evidence: The program owner stated that not all staff have been trained on the prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children.
Plan of Correction: Providers will take the training by 2/28/2022.

Based on observation, the license was not posted in a place conspicuous to the public.
Evidence: During the inspection, the license was not posted and was located on the floor behind a filing cabinet.
Plan of Correction: Document fell behind the cabinet during inspection while attempting to show copy of last inspection. It has been placed in a different bulletin board so the cabinet is not in the way as of inspection.

Based on interview, the center has not developed written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence: The program owner stated that they have not yet developed written procedures for preventing shaken baby syndrome or abusive head trauma.

Standard 8VAC20-780-40-M
Based on interview, the center has not established a current written list of all children's allergies, sensitivities and dietary restrictions.
Evidence:
1. Child #2 has a documented food sensitivity.
2. Child #5 has a documented food allergy.
3. The center owner stated that they do not have a list of each child's allergy, sensitivity and/or dietary restriction.
Plan of Correction: An 8" by 11" inch sheet has been posted in all three areas with names and restrictions for teacher visibility and reminder. Also, each teacher is aware.
