Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 1/20/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 17 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 4 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
7Evidence: The program owner stated that they have not yet installed a carbon monoxide detector.
Evidence:
1. Staff #1 (date of hire: 12/9/2020) TB screening is dated 1/6/2021.
2. Staff #2 (date of hire: 10/21/2021) TB screening is dated 11/12/2021
Evidence: Staff #3 (date of hire: 6/3/2019), Staff #4 (date of hire: 9/7/2020) and Staff #5 (date of hire: 9/7/2021) records do not contain documentation of the required Virginia Preservice orientation training. The program owner stated that not all staff have completed the required training within 90 days of employment.
Evidence: The program owner stated that not all staff have been trained on the prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children.
Evidence: During the inspection, the license was not posted and was located on the floor behind a filing cabinet.
Evidence: The program owner stated that they have not yet developed written procedures for preventing shaken baby syndrome or abusive head trauma.
Evidence:
1. Child #2 has a documented food sensitivity.
2. Child #5 has a documented food allergy.
3. The center owner stated that they do not have a list of each child's allergy, sensitivity and/or dietary restriction.
