Inspector Notes
Today an unannounced Renewal Inspection took place between the hours of 10:15am and 12:30pm. There were 2 rooms observed with a total of 16 children with 4 staff within the supervision guidelines. The children were observed transitioning to play outside, eating a morning snack and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
8Evidence:
1. Child #4 (21 months) most recent documented immunizations are dated from when the child was 14 months old)
2. Child #5 (21 months) most recent documented immunizations are dated from when the child was 10 months old.
Evidence:
1. Staff #1 completed 4 hours of training in 2022.
2. Staff #2 did not complete any training in 2022.
3. Staff #3 completed 4 hours of training in 2022.
4. Staff #5 did not complete any training in 2022.
Evidence: The center director confirmed that they did not have a working battery operated radio.
Evidence: There was no evacuation drill documented for December 2022. The program director confirmed that a drill was not practiced in December.
Evidence:
1. Child #3 was observed sitting in a high chair and was not buckled into the chair.
2. Child #4 was observed sitting in a high chair that did not have the buckles installed into the chair for the staff to buckle the child in.
Evidence: One bottle was observed in the infant refrigerator with no name or date.
Evidence: The current license states that no more than 4 children under the age of 2.5 years shall be permitted to be present in the infant classroom. There were 5 infants present in the classroom at the time of the inspection.
