Inspector Notes
Today an unannounced monitoring Inspection took place between the hours of 10:45am and 1:00pm. There were 2 classrooms observed with a total of 23 children with 6 staff within the supervision guidelines. The children were observed watching a movie while eating popcorn, eating . A complete inspection of the physical plant, children and staff records and emergency procedures were observed during this inspection. The program director stated that there are no medications currently kept on-site. There was an adequate number of staff with current DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

7Based on observation, unused electrical outlets did not have protective covers.
Evidence:
1. One electrical outlet in the large classroom behind the feeding chairs did not have a protective cover.
2. One outlet in the infant classroom under the window did not have a protective cover.
Evidence: During todays inspection, infants were observed in bouncing chairs, high chairs and in a stroller during the duration of the inspection. Infants were not given room for extensive movement. There was not a diversity of sensory and perceptual experiences offered and equipment and toys did not support large and small motor development.
Evidence: Staff #3 was observed working alone in the infant classroom. Center management confirmed that Staff #3 does not have current CPR certification.

Evidence: The toddlers were observed eating pizza and fruit for lunch. The preschoolers were observed eating pizza and salad. Both groups were not offered both a fruit and vegetable.
Evidence: Child #6 was observed walking and running around the large classroom while eating a slice of pizza.
Evidence: Child #7s bottles were not dated.
