Inspector Notes
A Subsidy Health and Safety inspection was conducted on 7/30/2024 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation and interview, children were not protected from materials that could be swallowed or present a choking hazard.
Evidence: 3 balloons were in a play area in use by infants. One infant attempted to put it in
their mouth.
