Inspector Notes

An unannounced inspection was initiated and completed on 5/6/2025, in response to a self report received by the licensing office on 5/12/2025 relating to allegations of supervision. The inspector reviewed 2 children?s records and 1 staff record on-site. This inspection included, document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Though not a part of the original self report, violations that were not reported were found related to staff training and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/3/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-245-H
Annual training shall be relevant to staff's job responsibilities and the care of children, and include signs and symptoms of child abuse and neglect and requirements for mandated reporters.

Based on record review, staff #1 had not completed training on signs and symptoms of child abuse and neglect and requirements for mandated reporters.
Plan of Correction: All staff have undergone trainings and meetings every two weeks to go over responsibilities, standards and supervision.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Staff responsible for supervising children did not ensure their care and protection. Two children were engaged in physical touching for 10 minutes, while staff was present, without staff knowledge, while staff was present in the room.
Plan of Correction: Continuous training, to go over marked areas and developed new plans for supervision including walks through the classroom every 10 minutes while children are napping and/or doing activities.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Children under 10 years of age were not in sight or sound supervision of staff, during nap time, staff had back to the children for 10 minutes and children were engaged in physical behaviors.
Plan of Correction: Visual signs and areas marked for teacher to take breaks or work during naptime so that they are always facing the classroom.
