During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 7/25/2025, as a part of the licensure period. The on-site inspection began at 12:10pm and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 13 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/4/2025. A POC submitted after this date will not appear on the public website.
Violations
5Child #3 and Child #4's immunization were not obtained by the center, prior to attendance. Child #3 was in care for 60 days, prior to immunization record being obtained, child #4 was in care 3 weeks, prior to obtaining immunization record.
Staff #2's updated screening was due 6 months ago.
Authorizations for diaper creams/ointments for children #2, #3 and #4 did not note whether there were any adverse reactions to diaper cream/ointments used.
1. Child #5's record did not contain name and phone number of child's physician;
2. Child #4's record did not contain names of persons authorized to pick up the child
3. Child #2, #3, #4 and #5's records did not have documentation of date of viewing proof of identity and age.

the parent and include authorization for emergency medical care should an emergency occur
when the parent cannot be located immediately unless the parent states in
writing an objection to the provision of such care on religious or other grounds; a statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and
a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
Child #5's record did not contain parental agreements.
