Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunizations obtained for Child #3 were dated 8 days after their first date of attendance. The record for Child #6, who has been attending the center for 11 months, did not contain documentation of immunizations.
Plan of Correction: Immunizations will be obtained prior to a child's first day. The center has requested immunization records from the parent for Child #6.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. The physical exam in the record for Child #6 was obtained 11 months after the child's first date of attenance.
Plan of Correction: The center will obtain physicals within 30 days of attendance.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside, are maintained in a clean, safe and operable condition. Peeling paint was observed in areas throughout the center, accessible to children. In the toddler room, peeling paint was observed underneath a window and at the bottom of a door. In the Pre-K room, peeling paint was observed in three window sills. In the Transitional K room peeling paint was observed on a column. In the School-age room, peeling paint was observed along the bottom of the walls throughout the classroom.
Plan of Correction: A work order has been requested.
Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of Child #2 had not been updated in over 15 months which is overdue by three months. The information in the record for Child #4 had not been updated in over two years.
Plan of Correction: The center will request the parents review the information.
