8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Inspector Notes

An unannounced subsidy health and safety inspection was conducted on 06/05/2024 from approximately 11:20 am until 12:05 pm. There were two children in care at the time of the inspection. The vendor was not present during the inspection. Required background searches, training certificates and household member information was reviewed. Emergency preparedness planning and supplies were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-170-A
Based on observation and interview, the vendor did not ensure compliance with the standards, and the terms of the vendor agreement.
Evidence:
1. The vendor was not present during the inspection.
2. Adult #2 confirmed the vendor was out of the country from 6/02/2024 for an undetermined amount of time.
Plan of Correction: Due to an emergency I left the country and went home because my mother had a bad accident and she broke her legs, she already had health issues which this one make it worse. She has no one to take care of her, she is under treatment as soon as she gets better and be able to take care of herself, I will return.

Based on observation and interview, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. There was no attendance available during the inspection.
2. Adult #1 could not find the attendance.
Plan of Correction: I always maintain and keep daily attendance sheet for the children I take care. As mentioned there are two more folders related to my work, which need to be organized and I will make sure it will be ready for future visits, unfortunately my sister in law could not find it. My plan is to keep all my necessary documents in one place to make it easy for visitors and avoid confusion in the future.

Based on observation, the vendor did not maintain, and keep at the family day home, written or electronic information for each enrolled child.
Evidence:
1. Child #2 did not have any information available during the inspection.
2. Adult #1 confirmed they could not find any information for child #2.
Plan of Correction: There is only one child under my care Farham, I always keep child information on paper but unfortunately my sister in law could not find it, but it was already there. there are other folders related to my work, I will make sure to organize my paperwork for the future visit. child #2 is not under my care, she is a family member.
