During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on December 16, 2024. The on-site inspection began at 12pm and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 26 children present and 7 staff. The inspector reviewed 6 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. The plastic truck on the playground has two cracks to the hood of the truck which poses as a pinching hazard to children in care.
Plan of Correction: The center responded with the following: The center will have the plastic truck repaired to ensure the children will not be hurt or harmed.
Standard 8VAC20-780-440-E
There shall be at least 12 inches of space between occupied rest mats. In the preschool room, 6 children that were napping did not have 12 inches of space between them.
Plan of Correction: The center responded with the following: Moving forward, all rest mats will be 12 inches apart.
Standard 8VAC20-780-440-J
There shall be at least thirty inches of space between service sides of occupied cribs where that space is the walkway for staff to gain access to any occupied crib. An occupied crib had two portable infant bouncers beside both service sides of the crib.
Plan of Correction: The center responded with the following: During the time of the inspection, both portable infant bouncers were moved and there was 30 inches of space on the service side of the occupied crib.
