Inspector Notes

Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on June 14, 2022 from 2:42 p.m. through 3:51 p.m. There were ten children in the care of the provider and one assistant for a total of eighteen points. Three children's records and the records for the provider, assistant and one household member were reviewed/updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Interactions between the provider and children were observed as positive and the children seemed comfortable with their daily routine and schedule.

Standard 8VAC20-800-150-D-1
Based on record review and discussion, the provider failed to ensure that assistant maintained a current certification in cardiopulmonary resuscitation (CPR).

Evidence:
1. Assistant Provider's cardiopulmonary resuscitation (CPR) certification expired on 11/2021.
2. Provider confirmed assistant's CPR certification is expired.
Plan of Correction: I have her scheduled to take the class in a couple weekends.

Based on record review and discussion, the provider failed to ensure that assistant maintained a current certification in first aid.

Evidence:
1. Assistant Provider's first aid certification expired on 11/2021.
2. Provider confirmed that the assistant's first aid certification expired.
Plan of Correction: I have her signed up to take a class in a couple weekends.
Standard 8VAC20-800-180-A
Based on a review of caregiver records, it was determined the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Evidence:
1. Provider's tuberculosis screening expired on 06/09/2022.
2. The provider confirmed the TB screening was expired.
Plan of Correction: I will schedule an appointment to get an updated TB screening next week with my doctor.
