Inspector Notes

A monitoring inspection was initiated on 12/12/2022 and concluded on 12/12/2022. There were 10 children present, ranging in ages from 2 years to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 2 staff records were reviewed/updated. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review, the provider failed to maintain written documentation of each caregiver's applicable education and programmatic experience, applicable first aid and CPR certification, orientation, annual training, and applicable medication training.

Evidence:
1. Caregiver #2's record lacked documentation of completed orientation training.
2. Caregiver #1 stated the training was completed but the documentation was not filled out.
Plan of Correction: I will be sure to fill out the orientation paperwork as soon as the assistant caregiver and substitute complete the orientation.
