Inspector Notes

An unannounced monitoring inspection was conducted on February 8, 2024 from approximately 4:30 pm to 5:30 pm. There were 22 children in attendance and a total of 4 staff present. 4 staff records and 5 children?s records were viewed. All areas of the center were observed including the cafeteria, gym, library, bathrooms, and the playground. The children were observed watching a movie, and doing homework while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 02/20/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on record review, observation, and interview, the center failed to obtain the results of a sex offender registry record check in any state that staff have lived in within the preceding five years..
Evidence: The record for staff #1, date of employment 05/04/2023, revealed that staff #1 lived in a state outside of Virginia in the preceding 5 years. When asked to electronically view the results of the sex offender registry from that state, administration stated that the results were requested but they could not be opened due to the electronic file being corrupted. Staff #1 was observed working in the center.
Plan of Correction: A new sex offender search has been completed and added to file.
Standard 8VAC20-780-190-A
Based on record review, the center failed to have a qualified program director.

EVIDENCE:

Staff #2, who stated they were the program director, does not meet the qualifications to be the program director.
Plan of Correction: An allowable variance has been submitted with plan 02/19/24. Staff will sign up for courses.

Based on record review, and interview, the center failed to keep at the center the name, address, and telephone number of a person to be notified in an emergency for each staff person.
Evidence:
1. The center did not have emergency contact information at the center for staff #3, date of employment 10/24/23 or for staff #4, date of employment 11/07/23.
2. Interview with the Director confirmed that the center did not have emergency contact information for staff #3 or staff #4.
Plan of Correction: Staff info sheet have been completed and filed.
