Inspector Notes
An unannounced subsidy health and safety inspection was conducted on February 8, 2024 from approximately 4:30 pm to 5:30 pm. There were 22 children in attendance and a total of 4 staff present. 4 staff records and 5 children?s records were viewed. All areas of the center were observed including the gym, cafeteria, library, bathrooms, and the playground. The children were observed watching a movie, and doing homework while the licensing inspector was on-site.
A complete licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection.
Violations were cited as a result of this inspection.
Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 02/20/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

1. The record for staff #3, date of employment 10/24/23, contained an expired CPR dated 02/16/2018. Current CPR was due on or before 01/24/24.
2. The record for staff #4, date of employment 11/07/23 did not contain documentation of CPR.
3. Interview with administration confirmed that staff #3 and staff #4 do not have current CPR.
Evidence:
