During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/29/2025. The on-site inspection began at 9:50am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. There were 54 children present and 14 staff. The inspector reviewed 7 children?s records and 5 staff records on-site.

This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six
months for children under the age of two years.

1. The immunization records for Child B (Age 2) indicates the child?s most recent immunizations were administered over 1 year 10 months ago.
2. The immunization records for Child C (Age 2) indicates the child?s most recent immunizations were administered over 2 years 3 months ago.
Plan of Correction: All parents will be reminded at least one month prior to when their child is due for a new immunization. The parents must receive a copy of the shot records the day of visiting the doctor and submit it to us immediately. The children will not be able to attend without receiving the new immunization records and if the doctor cancels their appointment, we will need to have documentation when the new date is and signed by the doctor.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

Child A with a diagnosed food allergy, did not have their emergency medication with them on their field trip. The medication was still onsite, and the child was off site on the field trip.
Plan of Correction: Staff has been retrained to have all medications on the field trips as well as a medical lock bag will be in our safety backpack.
