The center shall repeat background checks for all staff every five years. The record for staff #2 had an expired sworn statement.

The center must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #2, who had been employed for approximately two months, did not have a completed national criminal background check.

REPEAT VIOLATION
The center must request a search of the central registry prior to the employee's first day of employment.
Staff #3, who had been employed for approximately two months, did not have a central registry search requested.

The program is required to submit results of potable water testing to VDH. The program did not have evidence of submitting test results to VDH.

The following staff records shall be kept for each staff person: Two or more references as to character and reputation as well as competency were checked before employment.
The record for staff #3, staff #4 and staff #5 did not have documentation of references.

Each staff member shall submit documentation of a negative tuberculosis screening that has been completed within the last 30 calendar days of the date of employment. The tuberculosis screening for staff #3 was completed more than 30 days before employment.

REPEAT VIOLATION
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. A cabinet in the two year old classroom had cans of shaving cream with the label ?keep away from children? unlocked and accessible to children.

Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills. The center did not document evacuation drills for the month of October and November 2025.
