A subsidy health and safety inspection (SHSI) was conducted today to review supplemental health and safety requirements not covered in the licensing regulations. A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. Based on records reviewed and interview conducted, it was determined that the Provider has not completed the Virginia Annual Health + Safety training course. Evidence: There was documentation of completion, and the Provider reported that she had not taken this course.
