Inspector Notes
A subsidy health and safety inspection (SHSI) was conducted today to review supplemental health and safety requirements not covered in the licensing regulations. A licensing inspection was conducted on the same date, and the two inspections combined cover all SHSI requirements. One area of non-compliance of the supplemental subsidy health and safety requirements was found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Based on records reviewed and interview conducted, it was determined that a required annual subsidy training was not completed in the last 12 months. Evidence: There was no documentation that the provider completed the Virginia Health + Safety Training class, and the provider reported that she did not complete the training.
Plan of Correction: The provider stated that she would complete the class after the inspector sends the registration information.
