During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was conducted on 5/16/25. The on-site inspection began at 11:09am and ended at 12:26pm. The inspector reviewed compliance in the areas listed above. There were 3 children present and 3 caregivers; the point total was 5. The inspector reviewed 5 children?s records and 4 caregiver/household member(s) records while on-site. The inspection included document review, tour of the home and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/23/25. A POC submitted after this date will not appear on the public website.
Violations
11Substitute caregiver #2 began working on 4/17/25 and does not have a completed CRC. It was reported that the licensee has not requested a CRC.

Substitute caregiver #2 began working on 4/17/25 and does not have a completed CRC from the State listed on the sworn disclosure statement and a sex offender search. It was reported that the licensee has not requested these background checks.
The vehicle used to transport children is not tagged in Virginia and has not had a Virginia safety inspection.
The record for substitute caregiver #2 did not include date of employment; name, address and phone number of an emergency contact; a minimum of 2 written reference checks and documentation of the education and training required for a substitute caregiver.
The timesheets for substitute caregiver #1 and #2 have not been maintained on a daily basis. Both reported to be working daily since 4/17/25 and were working today however, the last timesheet entry for working for both was 5/8/25.
A cell phone and purse were observed lying in a crib reported to be the sleep space for Child #6.
There was no record of attendance for Child #7 who was in attendance today.
There was no record of enrollment for Child #7 who was in care today.
There was no documentation of the policy number for Child #6. The space on the form was left blank.
The immunizations on-file for Child #6 were dated July 2024 when the child was 3 months old.
