This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews. The Licensing Inspector conducted an investigation in response to a self report incident of behavior guidance on 9/2/2021. Interviews were conducted with staff. The information gathered during this investigation does not support the allegations; therefore no violations were cited.
