During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 05/14/2025 and completed on 05/29/2025 in response to a complaint received by the licensing office on 05/08/2025 relating to allegations of child abuse and neglect, and center programs. 7 staff records were reviewed. This inspection included documentation review, a tour of the facility, interviews, observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 06/05/2025. A POC submitted after this date will not appear on the public website.

COMPLAINT RELATED

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Staff A and staff B reported that staff #1 frequently used a harsh tone and demeaning language with the children. Child #2 was upset and getting cleaned up from a potty accident and staff #1 said in a harsh tone "you're coughing in my face, stop crying".
Plan of Correction: All staff have been provided refresher training on behavioral guidance including managing and responding to children and providing social and emotional support to children in all classrooms.

COMPLAINT RELATED

The following actions or threats thereof are forbidden: roughly handling a child.

Staff A and Staff B reported that staff #1 shoved child #1 into classroom #1 while stating "no, you're not listening. Go in here, you don't want to be in this classroom". It was reported that child #1 dropped to the ground and when staff #1 noticed a tour in classroom #1, they picked up the child and took them back to classroom #2.
Plan of Correction: Note- An employee listed above is no longer employed. The staff member stated that while moving the child to another classroom the child fell to the floor and began crying as the staff member was preparing to leave the child. The staff member returned the child to the classroom and the child stopped crying after entering the classroom. All staff have been provided refresher training on behavioral guidance including managing and responding to children and providing social and emotional support to children in all classrooms.
