The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #4, employed for more than 2 months, did not have documentation of requesting a criminal history check from one state.
Plan of Correction: Resolved on Date of Inspection by Submitting Request for KY. National Fingerprinting results for OBI and SO registry results are all satisfactory and have been on file since before the employee?s first day. Inspection identified that Kentucky was non-NFF and KY request was submitted immediately following inspection on 2/26/26.

Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes: Identity of the person conducting the drill; The date and time of the drill;The method used for notification of the drill; The number of staff participating; The number of children participating; Any special conditions simulated; The time it took to complete the drill; Problems encountered, if any.

The 2025 lockdown drill that was conducted was not documented with required information.
Plan of Correction: Corrected during inspection. The lockdown drill log included all required elements on the form except for the date and time, which were maintained on the center?s master calendar. The date and time were added directly to the drill log during the inspection, and the record is now complete.
