Inspector Notes

An unannounced inspection was conducted today from 4:30pm to 5:35pm. There were 5 children (8 points) in care of one caregiver. The children were observed playing outdoors, eating snack and leaving with parents for the day. There were no children in care with allergies or medications. 2 children's records, 1 caregiver's record, attendance, and emergency drill records were reviewed. Standard 22.1-289.035-A
Based on records review and staff interview, the family day home did not obtain repeat background checks every five years for all employees.
Evidence:
1. Caregiver # 1's most recent sworn disclosure statement was dated 6/29/18.
2. Caregiver # 1's most recent fingerprint results were dated 7/12/18.
3. Caregiver # 1's most recent central registry results were dated 8/13/18.
4. All 3 background checks were not repeated in five years.
Plan of Correction: The sworn statement has been updated and shared with the licensing inspector. The fingerprints and central registry will be submitted within ten days of the inspection date.
Standard 8VAC20-800-240-A
Based on observation, furnishings inside the family day home were not maintained in a safe and operable condition.
Evidence: The plexi-glass cover on the large table in the basement was broken in one corner, exposing sharp edges and posing an injury hazard to the children.
Plan of Correction: The plexi glass will be covered so that the sharp edges are not exposed.

REPEAT VIOLATION
Based on record review, the family day home did not maintain a written record of children in attendance each day.
Evidence: A review of the attendance records showed that the provider had not maintained written attendance for children in care after 11/3/23.
Plan of Correction: Attendance will be updated. It will be documented daily moving forward.
