Evidence:
1. The record for Staff #5 (date of employment 11/29/2023) did not contain documentation of a negative TB screening.
2. Staff #5 was counted in the staff-to-child ratio during the inspection.
3. A member of management confirmed this information during the inspection.
Plan of Correction: Going forward, we will ensure new staff start with TB screenings obtained within the required timeframe.

Based on review of five children?s records and interview, the center did not have a written allergy action care plan for one child diagnosed with a food allergy, to include instructions from a physician regarding the steps to take in the event of an allergic reaction.

Evidence:
The record for Child #1 (start date 09/25/2023) indicated the child has a food allergy. A food allergy action plan was in the record of Child #1. The food allergy action plan was not signed by a physician.
Plan of Correction: We returned the allergy action plan to have family obtain physician's signature.
