During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 3/3/2025 and completed on 3/3/2025. The on-site inspection began at 12:30pm and ended at 2:50pm. The inspector reviewed compliance in the areas listed above. There were twenty-one children present and four staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/18/2025. A POC submitted after this date will not appear on the public website.

The center is required to have employees undergo a background check every five years.

The most recent central registry check for Staff #2 was overdue by more than nine months.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #2, did not have a copy of their immunizations until 11 days after start date.

1. A bottle of Draino, 2 containers of Clorox wipes, a spray bottle of Windex, and a container of Lysol toilet bowl cleaner with warning labels, ?Keep out of reach of children,? were unlocked in the boy's bathroom cabinet, within reach of children.

Cots used by children during a rest period shall have linens consisting of a top cover and a bottom cover.
