Inspector Notes
A Subsidy Health and Safety inspection was conducted on 4/11/2022 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Based on a review of four staff records and interview, the vendor did not ensure one staff had obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. The record of staff #4 (DOH: 7/8/2019) contained a CPR certification dated 9/8/2019.
2. Administration acknowledged that the certification was expired.
