Inspector Notes
An unannounced inspection was initiated on 8/19/2024 and completed on 8/21/2024 in response to a complaint received by the licensing office on 8/14/2024 relating to allegations of physical plant, unsafe conditions and safety of children on the playground. This inspection included tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Based on observations and interviews, the center failed to ensure areas and equipment of the center, inside and outside, shall be maintained in a safe condition.
Evidence:
1. Tree branches were hanging over the fence into the play area and starting to cover some of the play equipment.
2. The branches were hanging over the fence almost touching the ground and in an area that was accessible to the children.
3. A declining tree was observed along the fence line.
