Inspector Notes

An unannounced renewal inspection was conducted today.
Children were observed playing on the playground, sleeping, during instructional time, circle time, singing and drawing.
Emergency supplies, first aid kits, emergency evacuation drills, menu, and handwashing was observed.
4 staff and 4 child records were reviewed.

Standard 8VAC20-780-140-A
Based on a review of records, one child record did not contain documentation of a completed physical within 30 days of attendance.
Evidence:
4 children's records were reviewed.
Child #3's record did not contain documentation of a completed physical. First date of attendance was documented as 8/14/23.
Plan of Correction: The family submitted the physical.
Standard 8VAC20-780-270-A
Repeat Violation
Based on observation, the center was not maintained in a clean, safe and operable condition.
Evidence:
One of three toilets (hallway bathroom) would not flush. The toilet had been used.
Plan of Correction: The water to that toilet was turned back on by maintenance.
Standard 8VAC20-780-350-B-1
Based on observation, a ratio of 1:4 (staff:children) was not maintained in the infant classroom.
Evidence:
Licensing Inspector (LI) observed a staff member leave the infant classroom which had 11 infants and 2 staff members when the LI stepped into the room to verify the number of staff present.
Plan of Correction: Additional training was given to staff on ratios. Training date 12/7/23.
Standard 8VAC20-780-350-B-3
Based on observation, a ration of 1:8 (staff:children) was not maintained in the 2's classroom.
Evidence:
When the Licensing Inspector (LI) and Licensing Administrator (LA) entered the Sky room, which contained children aged 2 years old, there were 19 children with 2 staff present.
Plan of Correction: Additional training given to staff on ratios. Training date 12/7/23.
Standard 8VAC20-780-420-E-1
Based on staff statement, and a review of the "Bright Wheel" system, there was no documentation of the amount of time spent on their stomachs for those infants who are awake and cannot turn over by themselves.
Evidence:
Staff stated the infants have "tummy time", but that they don't record the time.
Plan of Correction: While Tummy time is conducted for 30 mins per day per infant- it was not recorded in brightwheel and staff are reminded to add the time moving forward. [sic]
Standard 8VAC20-780-440-J
Repeat Violation
Based on observation, there was not 30 inches of space between the service side of a crib and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Evidence;
An occupied crib had approximately 4 inches of space between the service side of the crib and a glider/rocker.
Plan of Correction: The piece of furniture is removed to provide required space.
Standard 8VAC20-780-50-A
Based on observation, staff and child information was not kept confidential.
Evidence:
The Supplemental Information page, which identifies staff and child names, from the most recent inspection was posted to the public.
Plan of Correction: The last page is no longer posted.
Standard 8VAC20-780-510-I
Repeat Violation
Based on a review of medications, one medication present at the center did not have a prescription label attached.
Evidence:
Child #3's Epi-pen did not have a prescription label attached.
Plan of Correction: Parent has been informed to bring the prescription box.
Standard 8VAC20-780-550-P
Based on a review of injury reports, all required information was not documented,
Evidence:
4 of 5 injury reports provided for review did not contain:
1. Date and time of parent notification (2 reports),
2. Date of parent notification (1 report), and
3. Time of parent notification (1 report).
Plan of Correction: Additional training provided to staff on writing injury reports. The staff who wrote it incorrectly is no longer working at the T.E.A. Center.
