Inspector Notes
A subsidy inspection was initiated and concluded on February 16, 2023. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.

Based on record review the vendor failed to ensure that all staff who work directly with children have within 90 days of the date of employment current cardiopulmonary resuscitation (CPR) certification.
