Inspector Notes
An unannounced, on-site monitoring inspection was initiated on April 16, 2025 and completed on April 16, 2025. The on-site inspection began at 12:55pm and ended at 3:50pm. The inspector reviewed compliance in the areas listed above. There were 35 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (5/6/2025). A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The center did not have the most recent inspection violation notice posted.
Plan of Correction: All licensing postings have been updated with recent violation notice.
Standard 22.1-289.035-B-3
REPEAT VIOLATION
The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, staff #2 and staff #5 who have been employed for at least, and Staff #3 and #4, who had been employed for at least a month, did not have a central registry search requested. Staff #1, #2 and #3 were working alone with children.
Plan of Correction: All central registry for staff will be completed with new search and any new hire will be during initial hiring. We will forward the completed copy to our licensing inspector as soon as we receive the results.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The record for staff #3 did not have documentation of a completed tuberculosis screening. Staff #3 has been working with children for approximately three months.
Plan of Correction: Staff #3 has a TB screening, just didn?t turn it in once it was recorded. Staff #5 has contacted facility where test was held and they have given her documentation
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. The record for staff #2 who has been working at the center for over a year, did not have documentation of the completed orientation course.
Plan of Correction: All staff have completed an orientation explaining policies, standards, etc
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. The record for staff #3 did not have documentation of a completed staff orientation and has been working at the center for at least three months.

There shall be at least 12 inches of space between occupied cots, beds, and rest mats. Multiple occupied rest mats in the center had less than 12 inches of spacing in between them.
Plan of Correction: Sleeping space has been communicated with staff and measured before each nap time.
Standard 8VAC20-780-60-A
REPEAT VIOLATION
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: When applicable, work phone number and place of employment of each parent who has custody. The record for child #1 did not have the work phone number for both parents.
Plan of Correction: Child #1 records have been updated with work phone number for both parents.
Standard 8VAC20-780-70
REPEAT VIOLATION
The following staff records shall be kept for each staff person: Two or more references as to character and reputation as well as competency were checked before employment. The record for staff #1, staff #2, staff #3 and staff #4 had references that were obtained after employment.
Plan of Correction: All staff have now turned in all references with have documentation of letters of recommendation
