Inspector Notes
A monitoring inspection was conducted May 23, 2024. The licensing inspector was onsite, completing the inspection from approximately 3:30pm until 5:15pm. There were 27 children present, ranging in ages from five to 11 years, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five children's records and four staff members? records were reviewed. Additional staff record documentation was reviewed remotely on June 3, 2024.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within five business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the stand(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
2Based on a review of three medications and documentation review, the center did not ensure two medication authorizations were available to staff during the entire time the medication was effective for two children.
Evidence:
1. A non-prescription medication was observed at the center for Child #5. A medication authorization was not present at the center. Attendance rosters indicated Child #5 is currently attending the center.
2. A prescription medication was observed at the center for Child #4. A medication authorization was not present at the center. Attendance rosters indicated Child #5 is currently attending the center.
Based on observation and documentation review, the center did not ensure medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
A prescription medication for Child #4 was on site at the center. The medication was not labeled with the child's name, the dosage amount and the time or times to be given. Attendance rosters indicated Child #5 is currently attending the center.
