Inspector Notes

An unannounced monitoring inspection was conducted on 3/6/2024. At the time of entrance, 2 daycare children and 2 household children were in care with the provider. The record sample size consisted of 2 children's records and the provider's record and the household member's record. The children in care were observed engaging with the provider, participating in outdoor play. There have been no injuries since the last inspection and no medications are being administered at this time. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Based on record review and interview it was determined the provider did not ensure that the household member had the required central registry search completed every five years from the date of the initial search.
Evidence:
1. The record for the household member did not contain updated central registry search documentation, The documentation available was dated 4/25/18.
2. The provider confirmed that there was no updated central registry search documents available for the household member during this inspection.
Plan of Correction: Household member will submit an updated central registry search.
Standard 8VAC20-800-180-A
Based on record review and interview it was determined the provider did not obtain a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department for each household member.
Evidence:
1. The record for the household member did not contain an updated TB screening form. The available TB screening form was dated 8/5/21.
2. The provider confirmed that there was not an updated TB screening form.
Plan of Correction: Household member will get an updated TB screening.
