Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/26/24 and completed on 9/26/24. The on-site inspection began at 10:45am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. There were 15 children present and 2 staff. The inspector reviewed 5 children?s records and 4 staff records on site. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

The tuberculosis screening on file for staff #1 was completed and dated over 2 years ago. There was no renewed screening on file.
Plan of Correction: A renewed screening will be completed and placed within the staff file.
