During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/12/25 and completed on 8/12/25. The on-site inspection began at 8:45am and ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 staff. The inspector reviewed 5 children?s records and 4 staff records on-site.
This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Provide a sworn statement or affirmation disclosing whether he/she has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth prior to hire.

Staff #1 was newly hired and actively working without having a completed sworn statement at the time of inspection.
Plan of Correction: The sworn statement has been completed. To maintain compliance any new staff will complete the sworn statement prior to hire.
Standard 22.1-289.035-B-2
National finger printing results must be completed prior to hire.

Staff #1 was newly hired and actively working without having a completed finger print result at the time of inspection.
Plan of Correction: Finger print results have been received. To maintain compliance any new staff will have complete finger printing and receive results prior to hire.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff #3, who has been employed over 7 months has not yet completed the required 10 hour orientation course.
Plan of Correction: Staff will complete the required 10 hour training. For future compliance all new staff will complete this within 90 days of hire.
