Inspector Notes

An unannounced monitoring inspection was conducted on-site August 22, 2023. The provider was available during the inspection. There were 6 children present, ranging in ages from 6 months to 3 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 2 staff records, and 1 household member records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on a review of records and interview, the provider failed to ensure that each caregiver and every adult living in the family day home undergo the required repeat background checks every five years.
Evidence: 1. The record of staff #1 contained documentation of the most recent sworn disclosure statement dated 06/25/2018 and the most recent central registry background check was dated 08/20/2018. Staff #1 confirmed the background checks were not updated.

Based on a review of records and interview, the provider failed to ensure to obtain a tuberculosis screening for each caregiver every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence: 1. The record of staff #1 contained documentation of the most recent tuberculosis screening dated 07/19/2021.
2. The record of household member #1 contained documentation of the most recent tuberculosis screening dated 06/24/2021.
3. Staff #1 confirmed the updated tuberculosis screenings were not completed.
