Inspector Notes
An unannounced inspection was initiated on September 17, 2025 and completed on October 1, 2025 in response to a complaint received by the licensing office on August 20, 2025 relating to allegations of administration, personnel, physical health of caregivers, and caregiver training. The inspector reviewed 1 caregiver record. This inspection included document review, tour of the home, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 8, 2025. A POC submitted after this date will not appear on the public website.
Violations
8The family day home is required to obtain a completed sworn statement prior to the employee's first day of employment. Caregiver #1, who had been employed for over a month, had not completed a sworn statement.
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. The provider had not obtained a national criminal background check for caregiver #1, who had been employed for over a month.
The family day home is required to obtain background checks from any state in which the individual has resided in the preceding five years. The provider had not requested a search of an out-of-state sex offender check and an out-of-state child abuse and neglect registry check for caregiver #1, who had been employed for over a month.
Providers must maintain a record for each caregiver. There was no record for Caregiver #1 who has been caring for children for over a month.
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. There was no tuberculosis screening for Caregiver #1, who has been caring for children for over a month.
