This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a physical tour of the program.

A complaint inspection was initiated on 08/23/2021 and concluded on 09/17/2021. The director was contacted by telephone and an inspection was conducted. The inspector reviewed compliance in the areas of staffing and supervision, and special care and emergencies. A total of two staff records were reviewed. Three staff interviews were conducted relating to the allegations. The information gathered during the investigation does not support the allegation, so the complaint is determined to be "not valid".
