Inspector Notes
An unannounced monitoring inspection was conducted on 10/11/2022. There were 2 children present, ages 10 months- 15 months, totaling 8 points and 1 provider present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of diseases, emergencies, and nutrition. A total of 2 children?s records, 1 caregiver record and 1 household member records were viewed. The children were observed during meal time, diapering and independent play. The inspector discussed the following with the provider: firearms. The inspector arrived for the inspection at 10:30 am and departed the family day home at 11:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notices issued to provider.
Violations
1Evidence: The record for Child 1 did not contain the following: name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (only 1 contact was provided); written authorization for emergency medical care; and results of health examination for the child as required by the standard. Caregiver 1 confirmed this information was not available for review.
