Inspector Notes

A renewal inspection was initiated and concluded on 03/16/2022. There was 1 child in care and the provider's child, with the provider supervising. The inspector reviewed compliance in the areas of administration, household members, care of children, emergencies and nutrition. A total of 2 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, physical examinations obtained prior to attendance were not conducted within required timeframes for 2 of 2 children.

Evidence:
1. Child #1 began attendance on 2/21/2022. The physical examination in the record for Child #1 (age 5) was conducted on 10/29/2019, more than 12 months prior to attendance.
2. Child #2 began attendance on 2/21/2022. The physical examination in the record for Child #3 (age 4) was conducted on 7/20/2020, more than 12 months prior to attendance.
Plan of Correction: I will ask parent for new physical today.
