Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including
telephone contacts, documents review, interviews (if applicable) and a virtual tour of the
program (if applicable).

A monitoring inspection was initiated on Thursday, March 24, 2022 and concluded on Tuesday, March 29, 2022. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 126 children present with 22 staff supervising and 2 administrative staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of children's record, the center did not ensure to obtain documentation of a physical examination within 30 days after the first day of attendance for all children.

Evidence: In Child#4's record (start date 11/15/21), there was no physical examination documentation available for review during the inspection.
Plan of Correction: There was a physical examination dated 10/14/21. When scanned in digitally it was not sent as it was on the back of the form. Technological error.
