22.1 Background

Inspector Notes

A renewal inspection was initiated on and concluded on (date). There were 128 children present 17 staff supervising during the in person observation conducted by the inspector at the center. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A-1
Based on review of staff records and interview, the center did not ensure documentation of the tuberculosis screening was submitted at the time time of employment and prior to coming into contact with children.

Evidence: In Staff#3's record (date of hire: 4/11/22), the tuberculosis screening reviewed was dated 4/22/2022. Interview with staff confirmed the TB documentation available for review during this inspection.
Plan of Correction: The staff's prior TB test was from her old child care center and was completed in September 2021. She will be getting a new TB screening done on next week through the Virginia Department of Health.

Based on review of staff records and interview, the center did not ensure documentation for Tuberculosis (TB) screening was completed within the last 30 calendar days of employment.

Evidence: In Staff#2's record (date of hire 4/11/22), the TB screening reviewed was dated 09/28/21. Interview with staff confirmed the most recent documentation available for review during this inspection.
Plan of Correction: We will ensure that all new staff receive an updated TB screening prior to employment that is no more than 30 days old.
