Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 09/12/24 and completed on 09/13/24, as a part of the licensure period. The on-site inspection began at 11:45am and ended at 2:10pm. The inspector reviewed compliance in the areas listed above. There were 34 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. Additional documentation was reviewed electronically on 09/13/24. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

7The record of Staff #4, who has been employed at the center for over five months, did not have a completed sworn statement.
The central registry search results in the record of Staff #1, who is currently employed, was dated three months after their first day of employment.
The central registry search results in the record of Staff #2, who is currently employed, was dated two months after their first day of employment.
Staff #4 did not have central registry search results during the inspection and has been employed for over five months.
The TB screening in the record of Staff #3 was completed two days after the date of employment.
Staff #4 has been employed for over five months and does not have documentation of a negative TB screening.
Staff #2's updated TB screening was due three months ago. The updated screening has not yet been completed.
Staff #2 completed the course five months late.
Staff #4 has not completed the course and has been employed for over five months.
Staff #3 was alone with a group of seven, three-year-old children and was not certified in CPR or first aid.
Staff #1, who has been actively working at the center for the past 3 months, did not have any completed reference verifications on file.
