During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 02/13/2025 and completed on 02/18/2025, as a part of the licensure period. The on-site inspection began at 12:50pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. Additional documentation was reviewed electronically on 02/18/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
(Repeat Violation) The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The record of Staff #1, who has been employed for over three months, and Staff #2, who has been employed for over two months, did not have a completed sworn statement.
Plan of Correction: Staff were required to complete a sworn statement on 02/13/25. Moving forward, all new staff will be required to complete a sworn statement at the time of employment. Documentation will be maintained on file for each staff member.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child #5, who had been enrolled for over nine months.
Plan of Correction: The physical examination will be requested again.
Standard 8VAC20-780-160-C
(Repeat Violation) A tuberculosis (TB) screening is required every two years from the last screening.

Staff #5's updated TB screening was due over a month ago. The updated screening has not yet been completed.
Plan of Correction: Staff #5 will submit the repeat TB as soon as possible.
Standard 8VAC20-780-245-A
Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care.

Staff #3 obtained 12 of the 16 required hours in 2024.
Plan of Correction: The center was not aware that a training Staff #3 took does not count towards annual training. The center will not count that training moving forward and will ensure staff obtain at least 16 hours of annual training.
Standard 8VAC20-780-340-D
Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.

Staff #1, who did not meet lead teacher qualifications, was alone with a group of infant children the entire day on 02/13/2025.
Plan of Correction: The center will ensure a lead qualified staff member is regularly present with each group of children.
Standard 8VAC20-780-530-A
(Repeat Violation) At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff #1 was alone with a group of two infant children and was not certified in CPR or first aid.
Plan of Correction: CPR/First Aid class was scheduled for 02/17/2025. Staff #1 will not be left alone with a group of children until the certification is completed.
Standard 8VAC20-780-550-E
Shelter in place procedures shall be practiced a minimum of twice per year.

There was no evidence showing the center practiced the shelter in place procedures a second time in 2024.
Plan of Correction: Management will ensure shelter in place procedures are practiced and documented at a minimum twice per year.
