During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 02/13/2025 and completed on 02/18/2025, as a part of the licensure period. The on-site inspection began at 12:50pm and ended at 3:00pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. Additional documentation was reviewed electronically on 02/18/2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
7The record of Staff #1, who has been employed for over three months, and Staff #2, who has been employed for over two months, did not have a completed sworn statement.
There was no physical examination in the record of Child #5, who had been enrolled for over nine months.
Staff #5's updated TB screening was due over a month ago. The updated screening has not yet been completed.
Staff #3 obtained 12 of the 16 required hours in 2024.
Staff #1, who did not meet lead teacher qualifications, was alone with a group of infant children the entire day on 02/13/2025.
Staff #1 was alone with a group of two infant children and was not certified in CPR or first aid.
There was no evidence showing the center practiced the shelter in place procedures a second time in 2024.
