Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on Thursday, April 15, 2021 and completed on Friday, April 16, 2021. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 caregiver. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver and household member records submitted by the facility to ensure documentation was complete.

Standard 22VAC40-111-180-A
Based on review of the caregiver's record and interview, the provider did not ensure to obtain documentation of a tuberculosis screening every two years from the date of the first screening.

Evidence:
1. In the caregiver's record, the last documented tuberculosis screening was dated 7/10/2018.
2. In the household member record, the last documented tuberculosis screen was dated 7/27/2018.
3. Interview with the provider confirmed an updated tuberculosis report was not available for review during this inspection.
Plan of Correction: The provider will contact the health department today and schedule TB screening.
