Inspector Notes
A monitoring inspection was conducted on 05/12/2022, from 10:30am to 12:40pm. There were 24 children present and 5 staff. The Inspectors reviewed five children?s records and three staff records. The Inspectors observed staff and children?s interactions throughout the inspection. Children were observed during free play, circle time, lunch and feeding, and naptime. First aid kits were observed. Due to inclement weather, the playground was not observed during the time of the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the program director and assistant director at the exit interview.
Violations
5Evidence:
1) There is no record or documentation of an annual health inspection for the past year.
2) The center?s director, Staff #1, confirmed no inspection was conducted the past year.
The center?s director will contact the health department for the annual health inspection.
Evidence:
1) The infant room has an unlocked bottle of disinfectant cleaner beside the sink. There are five infants currently in care.
2) The one year old room with five children in care has an aerosol disinfectant spray can that on the label had a precautionary statement, ?Hazards to Humans,? that is unlocked on a shelf accessible to children.
3) The two year old room with seven children in care has an aerosol spray can of disinfectant and a bottle of disinfectant wipes in a cabinet that is unlocked.
4) The preschool room with seven children currently in care has two unlocked bottles of disinfectant wipes on the shelf in reach of children.
5) Staff #1 confirmed that the above cleaning solution all contained a hazardous cleaning material and that they were not appropriately stored and locked.
The center will ensure that all required hazardous substances are appropriately locked and not accessible to children.
Evidence:
1) The infant room had five children under the age of 16 months old, with one staff member to supervise. The youngest child in the infant room was six months old.
2) Staff #3 confirmed they are aware that the ratio for an infant room is one staff member for every four children. Staff #3 acknowledged the infant room was not within Staff-to-children ratio and group size requirements.
Evidence:
1) The seven children in the two year old classroom were observed in play time and transitioned to lunch during the time of the inspection. The children did not wash their hands prior to eating their lunch.
2) Staff # 2 confirmed that the children?s hands were not washed before eating lunch.
Evidence:
1) The center?s director, Staff #1, stated that the center did not implement a monthly practice drill, this year of 2022. The last fire drill was conducted December 2021.
